In the quiet, unincorporated area of Flowell, just west of Fillmore, Utah, a tragedy unfolded that left a family and community in mourning. On the evening of Thursday, July 24, 2025, deputies from the Millard County Sheriff's Office were called to a residence following a report of multiple deaths. Inside the home, they discovered the bodies of three individuals.
Among the deceased was Colleen Foutz Halsey, a 67-year-old White female.
Also found were her 28-year-old grandson, Jay Dean Stevens, and his 23-month-old daughter, Harper Helena Stevens. The initial investigation by the sheriff's office pointed to a grim conclusion. Both Colleen Halsey and Jay Stevens had sustained fatal gunshot wounds.
Further investigation revealed a heart-wrenching sequence of events. Authorities determined that Jay Dean Stevens was responsible for the death of his grandmother, Colleen. Evidence found at the scene indicated that after shooting Halsey, Stevens then turned the weapon on himself, resulting in a self-inflicted gunshot wound. A firearm consistent with the wounds was recovered at the residence. The cause of death for young Harper Stevens was not immediately released and was pending an autopsy.
The Millard County Sheriff's Office assured the public that there was no ongoing threat related to the incident.
