Kathy T. Nguyen, a Vietnamese woman, 61, died on October 31, 2001, in Manhattan after arriving at Lenox Hill Hospital in sudden respiratory distress that at first looked like a bad case of pneumonia or heart failure. In the days before she came in, she had been feeling progressively weaker, her chest heavy, her muscles aching; she was short of breath and coughing, and by the time she reached intensive care she was in respiratory failure and had to be intubated before investigators could get more ...
