On October 2, 2015, authorities in Indianapolis, Indiana, initiated a homicide investigation following the discovery of 41-year-old female Janet Lee Cook, who had sustained a fatal gunshot wound to the left side of her head. The incident occurred shortly after Cook had attempted to distance herself from a volatile relationship, having moved out of the home she shared with her boyfriend approximately two weeks prior to her death.
