Roman Francisco Aguilera Ortiz, a 4-year-old Hispanic boy from Coatesville, Pennsylvania, tragically died on February 28, 2022. The incident occurred when Roman found a loaded semi-automatic Glock model 43x pistol that had been left unsecured and unholstered on a table in his older brother Victor Lara-Ortiz's bedroom. The door to the room was open and unlocked. Roman accidentally shot himself in the head with the firearm, and he was pronounced dead at the scene.
