Azaveon Tavarus Raheem Cook, known as "Zay," tragically passed away on April 2, 2017, in Greenville, South Carolina. He was 21 years old at the time of his death. Azaveon was a 2015 graduate of Woodmont High School and was employed as a forklift driver at Reynolds Company. His death was a result of a shooting incident that occurred at the Boulder Creek Apartments.
