Jeffrey Dale Nichols, a 40-year-old air traffic controller, was last seen on the morning of June 8, 2004, in Salt Lake City, Utah. He had plans to meet his ex-wife and young son for breakfast at a McDonald's in Midvale, Utah, but he never arrived. Nichols reportedly called his ex-wife that morning, telling her he couldn't make the meeting, and he was never heard from again. His girlfriend reported him missing the following day.
