On March 15, 2021, Torpedoman's Mate 3rd Class Petty Officer Manuel Hernandez Julian Jr., a 23-year-old sailor assigned to the USS Charlotte submarine, was found dead at Pearl Harbor Naval Shipyard from an apparent self-inflicted gunshot wound. The incident occurred while he was on duty.
