On April 9, 2015, a series of events culminated in the death of 29-year-old Don O’Neal Smith Jr. The day involved a multi-county and multi-state police pursuit that ended in a hostage situation in Monon, Indiana.

The incident concluded with Smith being fatally shot, bringing a tragic end to the chase. Law enforcement officials were involved in the shooting that resulted in his death. During the confrontation, a hostage was also injured.
