On September 21, 2017, the Department of Child Safety received a report regarding 3 week-old Aniya
Carroll-Flores who was brought to the hospital with a head injury. Aniya later passed away due to the
injury. The Department filed a dependency petition with the Maricopa County Juvenile Court on
September 25, 2017 for the sibling. Subsequently a severance motion was filed for the father, Allen
