Janusz C. Kolodziejczyk, a 51-year-old male, was tragically killed on January 1, 2018, in Chicago, IL. The incident occurred in the Avondale neighborhood, where Kolodziejczyk was driving his Toyota Camry south on Pulaski Road. Around 2:20 a.m., his vehicle crashed into a fence in the 3900 block of West Cornelia Avenue.
