Kaitlyn Star Carroll-Peak, a 22-year-old woman from Mansfield, Ohio, tragically lost her life on March 8, 2017. Kaitlyn was known for her infectious smile and her deep love for her daughter, Kinsley. She was a dedicated mother and a compassionate person who always went out of her way to help others. Kaitlyn graduated from Ontario High School in 2013 and worked at Avita Hospital in Ontario.

The investigation into Kaitlyn's death revealed a series of failures by local law enforcement. Dakota Steagall, Kaitlyn's former boyfriend, was identified as the perpetrator. Steagall had a history of domestic violence and had threatened Kaitlyn and her family in the past. Despite these threats, law enforcement agencies failed to take appropriate action. Just days before Kaitlyn's murder, Steagall was seen outside her apartment, but he was not arrested. Eventually, Steagall abandoned the child with his grandmother and took his own life. The failures of the Richland County Sheriff’s Department and Mansfield Municipal Court Probation officials to arrest Steagall despite his probation violations and threats have been heavily criticized. Kaitlyn's family has expressed frustration and anger over the lack of action taken to protect her.
