Derrick P5 Johnson, a 23-year-old African American male, was fatally shot on the morning of October 30, 2012, in the South Side Woodlawn neighborhood of Chicago, Illinois. The incident occurred at approximately 9:13 a.m. in the 6200 block of South Eberhart Avenue. Johnson, who resided in the 5100 block of South Eberhart Avenue, was shot multiple times and was subsequently transported to John H. Stroger Jr. Hospital of Cook County. Despite the medical efforts, he was pronounced dead at 9:54 a.m.
