Christopher Scott McCormick, aged 45, was last seen on November 9, 2013, in Fort Wayne, Indiana. At the time, he was staying at the Glenbrook Rehabilitation and Skilled Nursing Center. On the day of his disappearance, McCormick walked away from the facility and has not been seen or heard from since.
