The unidentified male was operating a conversion van and traveling westbound with three additional passengers. The van was involved in a three vehicle motor vehicle accident. The van became fully engulfed in flames and all four occupants died as a result of the crash. 3 of the 4 occupants of the van were identified. The operator was known to an associate as Ronald Thomas. The four occupants of the van were carnival workers and employed by Amusements of America, Columbus OH.
