On February 5, 2019, Lloyd Wingfield, a 74-year-old resident of Cottonwood, Arizona, took his own life in a public place. The incident occurred at the Cottonwood Public Safety Building, which houses the local police department.

According to reports, a family member had contacted the Cottonwood police earlier that morning due to concerns about Wingfield's suicidal intentions. Despite this alert, Wingfield was able to enter the public safety building.
