Warren Gary Cook, a 56-year-old man from South Lebanon, Ohio, tragically passed away on April 21, 2014. His death was the result of a fatal shooting during a standoff with police officers at his home. The incident began when officers attempted to serve a domestic violence protection order. Cook, who had been drinking, repeatedly threatened the officers and refused to come out of his home.
