Charles H. Theis, a 76-year-old man, was last seen on October 14, 1975, in the wooded area near Military Road (FR 2199), approximately 0.45 miles north of Kentuck Lake Road in the town of Phelps, Wisconsin. He was in this area for an unknown reason when he disappeared. At the time of his disappearance, Charles was described as 5'9" tall, weighing between 150 to 175 pounds, with gray or partially gray hair.
