Johnnie Mae Miles, a 68-year-old woman, was last seen at her residence in Magna, Utah, on June 6, 2001. On that day, she reportedly left her home to go to the store and apparently became lost. This was the last time she was seen or heard from by her family.

The car was found crashed into willow trees near a creek off Cherry Creek Road, a dirt road located about twenty miles from Eureka in Juab County, Utah. Evidence at the scene suggested that Miles had gotten her vehicle stuck and had made unsuccessful attempts to free it.
A significant lead in the case came from a man who was herding cattle in the area in the early morning hours. He reported seeing a woman who matched Miles's description walking down Cherry Creek Road. The woman was seen carrying a purse and a brown paper bag. According to the witness, she turned off the road and began walking in the direction of Chicken Rock.
