She had a follow-up appointment scheduled for May 19, which she failed to attend. Sally was described as a white female with brown hair and hazel eyes, standing at 5'1" and weighing approximately 115 pounds. She had a distinctive tattoo on her right shoulder of a parrot on a branch in front of a half-moon, with a ribbon underneath bearing the word "Teko's". Additionally, she had a five to six-inch scar on her abdomen, possibly from a Caesarean section, and another tattoo on her back.

Her husband, who was incarcerated at the Ada County Jail at the time, reported her missing over Memorial Day weekend when she did not visit him as expected.
