Richard Phillips, a 32-year-old man, went missing on November 5, 1979, while aboard a small plane piloted by Michael Pfaff. Phillips and fellow passenger Albert Zeitler were flying over Lake Ontario when the aircraft encountered trouble and sent out a distress signal at 1:43 PM. The plane, a four-passenger aircraft, was never heard from again, and despite efforts to locate it, neither the plane nor the passengers were found.
