• Scar/mark: 1. On the left side of the frontal region, an inverted “Y” shaped scar is observed, measuring 1 x 1”. 2. Irregular scar on the anterior aspect of the left forearm, measuring 3 1/2” in length. 3. Scar on the medial aspect of the right leg, measuring 4 1/2” in length. 4. Irregular scar on the posterior aspect of the right leg, measuring 1 1/2 x 1”. 5. Two (2) scars on the anterior aspect of the left thigh, each measuring 1/2 inch at its longest dimension. 6. An irregular scar on the lateral aspect of the proximal third of the left leg, measuring 3 x 2 inches. 7 . A scar on the anterior aspect of the left leg, measuring 1 3/4 inches in length.
