Opal Shirley, a white female, died on September 13, 1994, at the age of 69, while under the care of licensed practical nurse Orville Lynn Majors at Vermillion County Hospital in Clinton, Indiana. Her death was one of several sudden fatalities that occurred during Majors' shifts, which later raised suspicions among hospital staff and investigators.
