Frederick LaGois, a 60-year-old male patient at Good Samaritan Hospital in West Islip, died on October 9, 1987, from Pavulon poisoning. The cause of death was determined through an autopsy conducted by Suffolk County Chief Medical Examiner Charles Hirsch, who stated that the toxicological testing and study of LaGois's medical records conclusively demonstrated the presence of Pavulon in his organs.
