Has anyone been looking into potential equipment malfunctions or safety protocol lapses on the USS Carter Hall around the time Taylor Machado went missing? It just seems strange to me that someone could go overboard unnoticed. I know ships are enormous and things happen, but it seems like crew safety measures would catch something like this or have safeguards in place. Or am I expecting too much precision with these procedures? Maybe someone here with Navy or maritime experience can shed some light on how these protocols typically work and what might lead to someone going unnoticed until it’s too late.
