I recently came across the case of Mark Doughty and the *Starbound* incident while doing some reading on maritime accidents, and something really stood out to me. The Coast Guard investigation found no clear reason why the *Virgo* failed to take evasive action and avoid the collision. It makes me wonder if there were any mechanical failures on board the oil tanker that they never disclosed, or if the crew just made a tragic error. I can't imagine how frustrating it must be for Mark's family and the other crew members’ loved ones to have such an open-ended conclusion. With no accountability, it feels like there's no closure possible for them. Does anyone here have more insight into whether it's common for such investigations to end without concrete findings?
