On June 13, 2008, Rafael Ortiz, a 40-year-old Hispanic male, was fatally shot in Chicago, Illinois. The incident occurred in the 2500 block of West Division Street in the Humboldt Park neighborhood. Ortiz was standing outside when an unknown assailant approached and opened fire, striking him multiple times.
