On October 27, 2015, Cooke embarked on a solo surf session at Waimea Bay, known for its formidable waves. It was the first significant swell of the season, with wave heights reaching 25 to 30 feet. Cooke, who had taken up night surfing in recent years, was last seen entering the water as dusk fell. His decision to surf at night, while unconventional, was characteristic of his adventurous spirit .
When Cooke failed to return home, his girlfriend reported him missing. Authorities found his truck parked near Saints Peter and Paul Mission Church on Kamehameha Highway, with his keys and dog still inside. His surfboard was later discovered near the Waimea River mouth, broken in half . An extensive search operation ensued, involving the U.S. Coast Guard, Honolulu Fire Department, and Ocean Safety crews. Despite covering approximately 8,900 square miles, the search was suspended after several days without finding any trace of Cooke .
Cooke's disappearance sent shockwaves through the surfing community. Known for his charisma and generosity, he had also faced personal challenges, including legal issues in the late 1980s.
