On September 16, 2014, David Gaines was shot and killed in Dayton, Ohio, on Huron Avenue. The shooting took place outside his home, and he was pronounced dead in the early hours of the next day after being transported to the hospital. According to the Montgomery County Coroner's Office, David Gaines died due to multiple gunshot wounds to his torso.
